Each submission must include:

• A completed Acord Application
• Currently Valued Loss Runs and Premium Information for the last 3 years
  
• Current NCCI Experience Rating Worksheet (**see footnote below)
• Supplemental Applications for:

Temporary Staffing Supplemental Application
PEO Supplemental Application
Logging Supplemental Application
Trucking Supplemental Application
Questionnaire for Housing Authorities (and other property management risks)

 Please also include:

• A detailed narrative explaining the operations of the insured
• A detailed description of losses in excess of $25,000
• A current Financial Statement for risks generating $250,000 or more in premium
• Any other information which might help in the underwriting/pricing of the risk

*The first submission received with sufficient information to underwrite the risk will be considered for a quote.  All other submissions for the same risk will be sent a “prior submission” declination letter.  In the event two or more submissions for the same risk are received in the same day’s mail, we reserve the right to determine which is more complete or offer a quotation on both (all) as per the individual application.

**Quotes will be issued on a contingent basis in the absence of a current NCCI Experience Rating Worksheet subject to adjustment upon receipt.
